Program Management: Make Safety Repeatable and Verifiable

A high-weight management domain focused on measuring performance, investigating incidents, managing change, building systems, leading people, controlling records and resources, and interpreting data.

Updated . Independent CSP-11 practice. Not BCSP-endorsed. No pass guarantee.

This domain contains 14 Master objectives and 38 detailed lessons. The objective layer organizes the blueprint; the lesson layer provides focused study and practice.

Domain overview

Program Management

Program Management turns individual safety activities into an organized system for performance. It connects leadership expectations, accountability, planning, culture, incident learning, change management, audits, documentation, resources, budgeting, project management, and data into a repeatable way of controlling risk.

CSP-style scenarios often hide a system weakness behind an immediate event. Repeated incidents, overdue actions, conflicting procedures, weak records, or bypassed controls may point to failures in ownership, Management of Change, assurance, communication, supervision, or resource allocation rather than a single employee mistake.

Strong programs are evidence-based. Policies and procedures matter, but implementation must be checked through field observations, leading and lagging indicators, audits, trend analysis, action-effectiveness reviews, and management decisions. Improvement depends on learning from weak signals as well as serious events.

What Domain 2 asks you to do

Turn safety activity into a managed system. Study the published tasks below, then open a class for the decision rule and exam traps.

  • Performance management
  • EHS culture
  • Incident investigation
  • Management of Change
  • System safety
  • Audits & standards
  • Leadership & data
  1. Compare organizational performance with established benchmarks and identify gaps.
  2. Use performance standards to determine an appropriate plan of action.
  3. Measure, analyze, and improve EHS culture.
  4. Select incident-investigation techniques, identify root causes, and apply corrective actions.
  5. Apply Management of Change before, during, and after a change.
  6. Recognize system-safety methods such as fault trees, FMEA, Safety Case approaches, and risk summation.
  7. Evaluate leading and lagging indicators and recognize EHS management and audit systems.
  8. Build plans, systems, policies, document-retention practices, budgets, leadership approaches, and project-management structures.
  9. Analyze and interpret exposure, release, sampling, statistical, probability, and Pareto data.

Decision rule

How to reason in Domain 2

When a problem repeats, identify the management process that should have prevented, detected, or corrected it. Assign ownership, use evidence to measure performance, and verify corrective-action effectiveness before closure.

Blueprint note: CSP-11 Domain 2 covers program performance, culture, incident investigation, corrective action, Management of Change, system safety, audits, leadership, resources, records, projects, and data.

Field case

Apply the domain under real constraints

A facility has three similar isolation events in six months. The losing answer is to retrain the involved employees after each event and treat the cases as unrelated. A program review finds confusing procedures, inconsistent supervisor verification, weak contractor controls, and corrective actions closed without effectiveness checks. The stronger response addresses those system weaknesses, assigns accountable owners, and measures whether recurrence risk falls.

Exam traps

Common reasoning errors to avoid

  • Treating a written policy as proof that the underlying management process is effective.
  • Responding to recurring failures with repeated retraining instead of investigating program and system causes.
  • Closing CAPA when tasks are complete without verifying effectiveness, sustainability, and ownership.

Domain 2 study questions

What does CSP-11 Domain 2 cover?
A high-weight management domain focused on measuring performance, investigating incidents, managing change, building systems, leading people, controlling records and resources, and interpreting data. SafePath maps original classes to this published area. It is not BCSP courseware.
How should I practice Domain 2?
Move from observations and incident data to measurable corrective action and improvement. Connect leadership, accountability, project management, budgeting, documentation, and EHS system requirements. Interpret safety data instead of treating a single metric as the whole picture. Then open the lesson list above and check the worked case before you move on. No practice product can guarantee a CSP result.
What makes a safety program manageable?
Clear objectives, responsibilities, resources, controls, records, performance measures, review mechanisms, and corrective-action ownership make the program repeatable and verifiable.
Why are leading and lagging indicators both useful?
Leading indicators can show preventive activity or control health, while lagging indicators show outcomes; using both provides better context than either alone.
When should management review intervene?
When trends, audits, incidents, resource gaps, overdue actions, or changing conditions show that objectives or controls may not be effective.
