RACI
A plant shutdown plan assigns both the maintenance manager and contractor supervisor as Accountable for the same isolation-release decision. Each expects the other to confirm readiness. Before work proceeds, how should you define the deliverable, decision owner and required verification?
Updated . Independent CSP-11 practice. Not BCSP-endorsed. No pass guarantee.
Decision rule
One person is Accountable. Many may be Responsible. Confusion here is a control failure.
RACI: Responsible does the work, Accountable owns the outcome, Consulted before, Informed after.
In 20 seconds
- One person is Accountable.
- Many may be Responsible.
- Confusion here is a control failure.
- RACI clarifies Responsible, Accountable, Consulted, Informed for each critical activity.
How to apply RACI
THEORY: Single-point accountability. Two Accountables is zero Accountables. RACI does not replace competence. CALC: None. FIELD: MOC with three ‘accountable’ names and no one who can say no. Name one Accountable with authority to stop start-up. ASSESS: Putting the CSP as Informed only on a high-risk change is a design choice to review.
Worked case
Wrong first move: add more names to share blame. Correct move: one Accountable, named Responsible doers, consult specialists before authorize.
Why the rule holds
Related: 22, 34, 31.
Same rule, different setting
A well-control matrix lists five Accountable names, including the CSP, the insurer, and “everyone.” A contractor is injured because nobody owned the barrier. You are the CSP. The question is what RACI allows.
A hoist matrix lists five Accountable names, including the CSP, the insurer, and “everyone.” A rigger is injured because nobody owned the lift. You are the CSP. The question is what RACI allows.
Picture / model
Map roles to specific outputs, test whether the decision owner has authority and resources, and define acceptance evidence at handover. Calculate a simple project network with explicit parallel work and show how a changed dependency changes available float.
Exam traps
Original SafePath contrasts for RACI. These are study traps, not recalled BCSP items.
| Trap | Correct read |
|---|---|
| R | Does the task |
| A | One owner — yes/no and resources |
| C | Advice with dialogue — typical EHS on operations |
| I | FYI, no veto |
Field move
The safety role is typically consulted. The insurer is not accountable.
Scenarios
Scenario 1 · Fundamental · CSP-11 D2.13
A LOTO row lists A: CSP, superintendent, insurer, “everyone.” Best correction?
Best action: One A: the line/process owner who can stop work and allocate isolation resources; CSP typically C; insurer not A.
Why: Evaluate the best response. The lockout row lists the safety practitioner, the superintendent, the insurer, and everyone as accountable. The best correction names one accountable person: the line or process owner who can stop work and allocate isolation resources. The safety role is typically consu…
Scenario 2 · Applied Scenario · CSP-11 D2.13
Who is typically Accountable for specifying a guard on a new press purchase?
Best action: The engineering/operations owner of the purchase, with EHS Consulted on hierarchy.
Why: Evaluate the best response. The best accountable person for specifying a guard on a new press is the engineering or operations owner of the purchase, with the safety role consulted on the hierarchy. The safety role reviews the specification. It is not the accountable purchaser. The vendor fabricate…
Scenario 3 · Applied Scenario · CSP-11 D2.13
You are the CSP reviewing a live decision at a Gulf Coast chemical terminal. Operations proposes a convenience path. In a RACI matrix, what distinguishes a consulted person from an informed person?
Best action: C is two-way influence before the decision; I is one-way after.
Why: Evaluate the best response. The best distinction is that consulted is two-way influence before the decision, and informed is one-way after. Reversing the two roles puts notice before input. Informed does not get a veto, and consulted is not a joint sign-off. Informed does not have to approve the re…
If the stem says this
| Stem signal | Best direction |
|---|---|
| two people think they are Accountable | name a single Accountable owner |
| a critical control has no Responsible party | assign Responsible and Accountable |
| contractor interfaces are unclear | write host/contractor RACI |
| two answers work | higher hierarchy / system / design |
| org chart changed last week | refresh RACI before assuming coverage |
Check before you leave
- ✓ RACI clarifies Responsible, Accountable, Consulted, Informed for each critical activity.
- ✓ One Accountable owner — not a committee fog.
- ✓ Responsible does the work; Accountable owns the outcome.
- ✓ Gaps and overlaps in RACI create missed critical controls.
- ✓ Update RACI after MoC and reorganizations.
- ✓ Contractors need explicit RACI with the host.
