ISO 19011
A fabrication shop schedules an internal audit one day before a customer visit. The production superintendent offers to audit the department they manage, and the proposed method is a binder checklist with no field observations. As the CSP, decide how ISO 19011:2026 should change the audit plan so the result is objective, risk-based and supported by traceable evidence.

Decision rule
An audit collects evidence against criteria. A tour is not an audit.
ISO 19011: competence, independence, evidence, findings. A finding needs criteria, condition, and evidence.
In 20 seconds
- An audit collects evidence against criteria.
- A tour is not an audit.
- ISO 19011 guides auditing management systems: principles, managing a program, conducting audits, competence.
- Audit evidence must be objective and sampled against criteria.
How to apply ISO 19011
THEORY: Audit criteria vs observed condition. Nonconformity is a failed requirement with proof. Opportunity for improvement is not a soft nonconformity unless the criteria demand it. CALC: None. Sampling of records is not a statistical claim unless the stem gives n. FIELD: Auditor writes ‘culture feels weak’ with no criterion. Rewrite as evidence against a participation or competence clause, or drop it. ASSESS: Auditing your own paycheck process without independence is the trap.
Worked case
Wrong first move: close the audit with opinions. Correct move: each finding cites clause, condition, and record or observation.

Internal “audit” on the platform is a 400-line checklist of posters, done by the area superintendent the day before the registrar. You are the CSP. The question is what ISO 19011 actually asks an audit to be.

Picture / model
Plan a management-system audit using ISO 19011:2026 guidance: define objectives, scope and criteria; manage competence, confidentiality and objectivity; sample according to risk; gather traceable evidence; report fair findings; and state conclusions no broader than the evidence supports.
Exam traps
Original SafePath contrasts for ISO 19011. These are study traps, not recalled BCSP items.

| 45001 / Z10 | OHS system being audited |
| 14001 | EMS being audited |
| Inspection / walkthrough | May feed an audit; not automatically a 19011 audit |

Scenarios
Scenario 1 · Fundamental · CSP-11 D2.08
You are the CSP reviewing a live decision at a grain elevator maintenance day. Operations proposes a convenience path. Which description of ISO 19011 is accurate?
Best action: Guidance on auditing management systems — principles and process — not the SMS itself.
Why: Evaluate the best response. The best description is guidance on auditing management systems: principles and process, not the system itself. This item does not quote that guidance. It is not an occupational system that replaces the occupational standard. It is not an environmental aspect standard. I…

The maintenance superintendent whose bonus is uptime audits LOTO in their own shop using binder tabs only. 19011 issues?
Best action: Independence and evidence: do not audit your own bonus; observe try-out in the field.
Why: Evaluate the best response. The maintenance superintendent whose bonus is uptime audits lockout in their own shop using binder tabs only. The best issues are independence and evidence. Do not audit your own bonus. Observe try-out in the field. Internal audit does not always mean the boss of the sho…
Scenario 3 · Applied Scenario · CSP-11 D2.08
Risk-based audit time is scheduled 90% on lunchroom posters and 10% on the open mixer. Problem?
Best action: Risk-based approach failed: high-energy operational control deserved the sample.
Why: Evaluate the best response. Ninety percent of the audit time is on lunchroom posters and ten percent on the open mixer. The best reading is that the risk-based approach failed. The high-energy operational control deserved the sample. Posters are not the leading reason to spend the sample there. Equ…
If the stem says this
| Stem signal | Best direction |
|---|---|
| auditors only tour offices and skip high-risk operations | risk-base the plan to sample critical controls |
| a finding lacks evidence | do not issue — gather objective evidence |
| management asks to delete a major finding | report accurately — ethics over comfort |
| two answers work | higher hierarchy / system / design |
| competence for the technical process is missing | add competent auditors/technical experts |
