ISO 45001
A plant wants ISO 45001 certification before a customer audit. Managers rush to complete procedures, while two mixer interlocks fail proof tests and operators say the new changeover method is unworkable. You are the CSP. Decide what PDCA requires beyond a certificate project.

Decision rule
ISO 45001 is a PDCA management system. A certificate on the wall is not control of risk.
Context, leadership, planning, support, operation, performance evaluation, improvement. Worker participation is a requirement, not a poster.
In 20 seconds
- ISO 45001 is a PDCA management system.
- A certificate on the wall is not control of risk.
- ISO 45001 is a management-system framework: context, leadership, planning, support, operation, evaluation, improvement.
- Worker participation and consultation are required elements — not optional posters.
How to apply ISO 45001
THEORY: ISO 45001 uses the HLS PDCA cycle. Hazards and OH&S risks are planned (clause 6) and controlled in operation (clause 8). Audit and management review close the loop. CALC: None. Do not treat certificate age as a risk score. FIELD: Certified site with no method for workers to stop a job. The gap is participation and operational control, not another policy paragraph. ASSESS: Buying a consultant binder before mapping significant risks is the wrong first move.
Worked case
Wrong first move: frame the certificate after a recordable. Correct move: reopen the risk assessment, restore the missing operational control, review it with workers.

Same rule, different setting
The registrar arrives at the gas plant in six weeks. The OIM wants a binder and a kickoff. You are the CSP. The question is whether ISO 45001 is a PDCA management system or a certificate hunt.

Picture / model
Apply ISO 45001 as a functioning occupational health and safety management system: leadership and worker participation inform planning, operational controls are implemented and supported, performance is evaluated with evidence, and failures drive correction and continual improvement.
Exam traps
Original SafePath contrasts for ISO 45001. These are study traps, not recalled BCSP items.
| Trap | Correct read |
|---|---|
| Clause 5 Leadership | Line leaders accountable; EHS enables — not CSP as the A |
| Clause 8 Operation | Hierarchy, MoC, contractors, emergency — the Do |
| Clause 9 Performance evaluation | The Check — monitoring, audit, management review |
| ISO 14001 | Environment — do not run OHS on aspect scoring alone |
Field move
A certificate ceremony is not the cycle. This item does not quote the standard.
Scenarios
Scenario 1 · Fundamental · CSP-11 D2.08
You are the CSP reviewing a live decision at a bridge deck pour laydown yard. Operations proposes a convenience path. Which sequence best represents the ISO 45001 management-system cycle?
Best action: Plan (risks, legal, objectives) → Do (operation, hierarchy, MoC) → Check (monitor, audit, review) → Act (CAPA, improve).
Why: Evaluate the best response. The best sequence for the named occupational health and safety management cycle is plan the risks, legal requirements, and objectives; do the operation, the hierarchy, and change control; check by monitoring, audit, and review; then act with corrective action and improve…
Scenario 2 · Applied Scenario · CSP-11 D2.08
Clause 8 operational control: an open mixer, substitution still possible. Draft control is a respirator program. Best 45001-consistent Do?
Best action: Apply hierarchy: substitute or enclose, then admin/PPE for residual.
Why: Evaluate the best response. Operational control is the question, the mixer is open, and substitution is still possible. The draft control is a respirator program. This item does not quote the named standard. The best do-step applies the hierarchy: substitute or enclose, then administrative controls…
Scenario 3 · Applied Scenario · CSP-11 D2.08
You are the CSP reviewing a live decision at a grain elevator maintenance day. Operations proposes a convenience path. Where does internal audit fit in the plan-do-check-act cycle?
Best action: Check — performance evaluation, feeding Act.
Why: Evaluate the best response. The best place for internal audit in the cycle is Check. It is performance evaluation that feeds Act. Plan is where the system is designed. The audit looks at whether that plan is working. Do is operation. An audit does not itself install the guards. Putting audit outsid…
If the stem says this
| Stem signal | Best direction |
|---|---|
| workers are excluded from hazard consultation | restore participation — required element |
| only a manual exists with no operational controls | implement operation and evaluate performance |
| a higher control is open in planning | select it under hierarchy expectations |
| two answers work | higher hierarchy / system / design |
| management review never sees audit findings | feed evaluation into improvement |
Check before you leave
- ✓ ISO 45001 is a management-system framework: context, leadership, planning, support, operation, evaluation, improvement.
- ✓ Worker participation and consultation are required elements — not optional posters.
- ✓ Risk and opportunity planning must drive operational controls.
- ✓ Documents without operation are nonconformity waiting to happen.
- ✓ Internal audit and management review close the PDCA loop.
- ✓ Hierarchy of controls is expected inside operational planning.
