FMEA
A packaging-line FMEA ranks “operator misses a solvent leak” at RPN 432. A camera drops the detection score and the spreadsheet looks much better, but the leak can still reach a hot surface. Your decision: did risk control improve enough, or did only one ordinal rating change?

Decision rule
RPN ranks rows. It is not a probability and not permission to skip a higher control.
RPN = S x O x D. Use it to order work. Combinations of barriers belong on FTA or LOPA, not inside one RPN.
In 20 seconds
- RPN ranks rows.
- It is not a probability and not permission to skip a higher control.
- FMEA ranks failure modes by severity, occurrence, and detection (RPN or action priority).
- High severity still demands action even if occurrence looks low.
How to apply FMEA
THEORY: FMEA scores severity, occurrence, detection for a failure mode. Detection of PPE donning must not outrank an open substitution. CALC: S=8, O=5, D=4 → RPN=160. TI-30XS: 8x5x4=. Compare RPNs only inside the same scoring scheme. FIELD: Highest RPN is ‘operator fails to don respirator’ while a substitute chemical is still approved. Fund substitution, not a better cartridge. ASSESS: Treating RPN as risk acceptance is the trap.
Worked case
Wrong first move: buy cartridges because that row had the biggest RPN. Correct move: keep hierarchy above RPN; use RPN only to sequence remaining work.
Why the rule holds
Related: 02, 23, 86.
Same rule, different setting
A wellhead FMEA lists “operator fails to don respirator,” RPN 720, and funds a better cartridge. The treatment chemical can still be substituted. You are the CSP. The question is what RPN is allowed to decide.
A coating FMEA lists “operator fails to don respirator,” RPN 720, and funds a better cartridge. A non-isocyanate spec is still allowed. You are the CSP. The question is what RPN is allowed to decide.
Picture / model
Use FMEA to trace how an item or process step can fail, what effects follow, why the failure can occur, and which controls exist; then use transparent local ratings to prioritize treatment without mistaking an RPN for measured risk or allowing score reduction to substitute for hazard control.
Exam traps
Original SafePath contrasts for FMEA. These are study traps, not recalled BCSP items.
| Trap | Correct read |
|---|---|
| High RPN | A ranking flag — still apply hierarchy to the action |
| Lower D | Better detection, not a lower hazard |
| FTA | Top-down combinations; no S×O×D |
| Bowtie / safety case | Barrier demonstration, not a component spreadsheet |
Field move
RPN = 8 × 7 × 9 = 504.
Scenarios
Scenario 1 · Fundamental · CSP-11 D2.06
You are the CSP reviewing a live decision at a plastics compounding turnaround. Operations proposes a convenience path. An FMEA rates severity 8, occurrence 7, and detection 9 on the same 1–10 scale. What is the RPN?
Best action: 504 (product).
Why: On one consistent scale, RPN = S × O × D = 8 × 7 × 9 = 504. The sum 8 + 7 + 9 = 24 is a different statistic. Severity alone, or severity times occurrence while dropping detection, does not use the three ratings the stem supplies. The product is not a legal risk limit.
Scenario 2 · Applied Scenario · CSP-11 D2.06
Worst RPN is “operator fails to don respirator.” Substitution of the isocyanate is still open. Best action?
Best action: Specify the substitute; RPN must not outrank a still-open higher control.
Why: Evaluate the best response. The worst score is an operator who does not don a respirator, and substitution of the isocyanate is still open. The best action specifies the substitute. The ranking must not outrank a still-open higher control. Better cartridges follow a ranking that skipped the open su…
Scenario 3 · Applied Scenario · CSP-11 D2.06
Two independent SIS loops must both fail for a catastrophic release. The team starts a DFMEA of each transmitter and never builds a tree. Problem?
Best action: Wrong tool: the top event is an AND combination — use FTA (or bowtie/safety case), not RPN alone.
Why: Evaluate the best response. Two independent loops must both fail for a catastrophic release. The team reviewed each transmitter and never built a tree. The best reading is the wrong tool. The top event is an AND combination, so use a fault tree or a bowtie, not a ranking alone. A parts review does…
If the stem says this
| Stem signal | Best direction |
|---|---|
| severity is extreme but occurrence is scored low | still prioritize action on severity |
| only detection is improved while prevention is open | prefer prevention/design controls |
| FMEA is filed after startup with no actions | reject — FMEA must drive change |
| a higher hierarchy control is still open | do not let RPN ratify PPE-first |
| two answers work | higher hierarchy / system / design |
Check before you leave
- ✓ FMEA ranks failure modes by severity, occurrence, and detection (RPN or action priority).
- ✓ High severity still demands action even if occurrence looks low.
- ✓ Detection controls are not as strong as prevention controls.
- ✓ FMEA must be able to change design or process — not only document risk.
- ✓ Update FMEA after MoC; stale FMEA is false assurance.
- ✓ Do not use FMEA RPN to justify skipping a still-open higher hierarchy control.
