PSM MoC + PSSR Integration
A high-high level trip was bypassed during a weekend repair on a covered process. Operations wants product in the line Monday morning. Decide first whether the change is replacement-in-kind or requires MOC, then decide whether the modification also triggers PSSR before highly hazardous chemicals are introduced.
Updated . Independent CSP-11 practice. Not BCSP-endorsed. No pass guarantee.
Decision rule
Advanced MOC includes temporary, emergency, and software.
A control-logic change is a change.
In 20 seconds
- Advanced MOC includes temporary, emergency, and software.
- MoC before install when chemicals, technology, equipment, procedures, or facilities change risk.
- Like-in-kind is narrow; risk change is MoC.
- When PSSR is triggered, verify design, procedures, applicable PHA/MOC requirements and training before startup.
How to apply PSM MoC + PSSR Integration
THEORY: PSSR before restart. Temporary expiry is a hard date. CALC: None. FIELD: Bypass ‘until Monday’ still in on Friday week two. Close or re-authorize. ASSESS: Like-for-like when firmware changed is the trap.
Worked case
Wrong first move: Leave the bypass. Correct move: Authorize, expire, verify after.
Why the rule holds
Related: 22, 87.
Same rule, different setting
A refinery contractor swaps a pump seal system for a different seal plan and adds temporary tubing on a covered process skid. The parts fit, but the leak-control basis changes. Treat the change through MOC and test whether the PSI change is significant enough to trigger PSSR before startup.
During a petrochemical turnaround, a construction crew installs a temporary bypass spool around a control valve so commissioning can stay on schedule. The superintendent calls it temporary and wants startup tonight. Temporary duration does not erase MOC; startup waits for the applicable readiness reviews.
Picture / model
Covered-process change → replacement-in-kind test → MOC before implementation → update affected PSI/procedures and inform/train affected personnel → determine PSSR trigger → complete startup verification before introducing highly hazardous chemicals.
Exam traps
Original SafePath contrasts for PSM MoC + PSSR Integration. These are study traps, not recalled BCSP items.
| Trap | Correct read |
|---|---|
| Like-in-kind | True form/fit/function/risk match — narrow |
| MoC | Authorize the change before install |
| PSSR | Required review for new facility or qualifying modification |
| Mechanical integrity | Inspect existing equipment — not a design authorization |
| Temporary change | Time-limited MoC with restoration |
Field move
Field decision: Confirm design match, procedures/PSI, applicable new-facility PHA or modified-facility MOC requirements, and training before startup.
Scenarios
Scenario 1 · Fundamental · CSP-11 D1.02
What must a pre-startup safety review confirm before a modified covered process starts?
Best action: Confirm design match, procedures/PSI, applicable new-facility PHA or modified-facility MOC requirements, and training before startup.
Why: Evaluate the best response. Pre-startup safety review and management of change confirm that the installed design, procedures, information, and training match before startup. A different seal, setpoint, software trip, or temporary hose is a change. A scheduled relief inspection with no design change…
Scenario 2 · Applied Scenario · CSP-11 D1.02
A pump is swapped to a different seal plan. Operations calls it like-in-kind. Best classification?
Best action: MoC — form/fit/function or risk changed.
Why: Evaluate the best response. Pre-startup safety review and management of change confirm that the installed design, procedures, information, and training match before startup. A different seal, setpoint, software trip, or temporary hose is a change. A scheduled relief inspection with no design change…
Scenario 3 · Advanced Engineering · CSP-11 D1.02
Operations wants a weekend bypass around a high-level trip with a radio waiver. Best CSP action?
Best action: Refuse the defeat or engineer an independent layer; emergency MoC is still MoC, not a radio waiver.
Why: Evaluate the best response. Pre-startup safety review and management of change confirm that the installed design, procedures, information, and training match before startup. A different seal, setpoint, software trip, or temporary hose is a change. A scheduled relief inspection with no design change…
If the stem says this
| Stem signal | Best direction |
|---|---|
| risk, metallurgy, logic, or capacity changed | open MoC — not like-in-kind |
| unit ready to start with open punchlist | block startup; finish PSSR |
| interlock defeat requested | refuse or engineer an independent layer |
| already running on a redline | shut down and complete the gate |
| training scheduled next quarter | not PSSR-ready |
| two answers work | higher hierarchy / system / design |
Check before you leave
- ✓ MoC before install when chemicals, technology, equipment, procedures, or facilities change risk.
- ✓ Like-in-kind is narrow; risk change is MoC.
- ✓ When PSSR is triggered, verify design, procedures, applicable PHA/MOC requirements and training before startup.
- ✓ Emergency MoC is still MoC with a time limit.
- ✓ MI does not authorize a new design.
- ✓ An open punchlist blocks startup.
