LOTO Verification / Group-Shift Mastery
A mixer jams with the guard open. Production proposes a spotter and a brief jog because shift change is near. Before anyone reaches into the danger zone, decide whether servicing exposes workers to unexpected energization, startup, or stored energy; if it does, apply the energy-control procedure and verify isolation.
Updated . Independent CSP-11 practice. Not BCSP-endorsed. No pass guarantee.
Decision rule
Group and shift LOTO keep each person’s protection attached.
A supervisor pocket key is not group lockout.
In 20 seconds
- Group and shift LOTO keep each person’s protection attached.
- Service with exposure requires full LOTO.
- Isolate every energy type, not only electrical.
- Each person applies a personal lock; group box multiplies locks.
How to apply LOTO Verification / Group-Shift Mastery
THEORY: Personal lock on the hasp or box. Shift change transfers, it does not share. CALC: None. FIELD: Crew of six, one shop lock. Issue personal locks. ASSESS: ‘We trust the lead’ is the trap.
Worked case
Wrong first move: One lock for all. Correct move: Each authorized person locks; transfer at shift change.
Why the rule holds
Related: 05.
Same rule, different setting
A process pump seizes after a trip. Mechanics must remove the coupling guard while suction pressure, electrical energy, and possible shaft movement remain. A radio watch and one operations lock are not enough: identify every hazardous-energy source, use the group procedure, verify isolation, and preserve protection through shift transfer.
A batch-plant mixer jams during a night pour. The crew wants one gang lock and a spotter while mechanics clear the paddles. Treat the jam as hazardous-energy servicing: isolate electrical and stored mechanical/gravity energy, use accountable group LOTO, and transfer protection without a gap at shift change.
Picture / model
Prepare → shutdown → isolate all energy sources → apply lockout/tagout devices → control stored/residual energy → verify isolation/deenergization → service. Group work: personal devices on the group mechanism; shift change: orderly transfer that preserves continuous protection.
Exam traps
Original SafePath contrasts for LOTO Verification / Group-Shift Mastery. These are study traps, not recalled BCSP items.
| Trap | Correct read |
|---|---|
| Spotter during a live inch | Administrative — not isolation |
| Personal lock plus try-out | LOTO sequence |
| One departmental lock overnight | Not each-person control |
| Cord-and-plug in exclusive control | Narrow exception — not for hard-wired mixers |
| Group box with each lock | Group LOTO done right |
Field move
Exposure requires full LOTO: isolate every energy, personal lock, dissipate, verify by try-out. Group and shift work multiply personal locks; they do not replace them. A spotter is not a lock. Verify the critical step before accepting a lower-tier control.
Scenarios
Scenario 1 · Fundamental · CSP-11 D1.03
After applying locks, how should an authorized worker verify that hazardous energy is controlled before work begins?
Best action: Try a normal start / prove zero energy on the isolated circuit.
Why: Core Rule: Verify is a test, not a pause. Calculation Steps: None — classify the last sentence, then apply the rule. Standards Cited: 1910.147-style sequence. Why Each Distractor Fails: Try a normal start / prove zero energy on the isolated circuit.: Correct: try-out / prove zero. | Read the proced…
Scenario 2 · Applied Scenario · CSP-11 D1.03
Day shift is leaving in twenty minutes. Night shift will continue the same isolation. Correct transfer?
Best action: Incoming workers apply their locks before outgoing locks come off.
Why: Evaluate the best response. Lockout and tagout is verified by a current try-out that proves zero energy for the people who are exposed. Each person needs their own lock. Stored air, gravity, and backfeed are isolated before entry. Cord-and-plug exclusive control does not transfer to a hard-wired ma…
Scenario 3 · Applied Scenario · CSP-11 D1.03
Host isolated a skid and applied a group lock. A contractor will enter. Best contractor action?
Best action: Verify isolation and apply a personal lock to the group device.
Why: Core Rule: Contractors still verify and personally lock. Calculation Steps: None — classify the last sentence, then apply the rule. Standards Cited: Multi-employer LOTO. Why Each Distractor Fails: Trust the host verbal all-clear and start.: Verbal is not a lock. | Verify isolation and apply a perso…
If the stem says this
| Stem signal | Best direction |
|---|---|
| hands are in the machine or the guard is open | require full LOTO |
| multiple workers share one isolation point | each applies a personal lock |
| the stem offers inch-mode or a spotter | reject the exception |
| shift change is in the last sentence | incoming locks on before outgoing locks off |
| only the breaker is locked | look for pneumatic, thermal, or gravity |
| two answers work | higher hierarchy / system / design |
Check before you leave
- ✓ Service with exposure requires full LOTO.
- ✓ Isolate every energy type, not only electrical.
- ✓ Each person applies a personal lock; group box multiplies locks.
- ✓ Verify zero energy by try-out; a smooth handle is not a test.
- ✓ Shift change transfers personal locks before outgoing locks come off.
- ✓ Cord-and-plug and minor servicing are narrow exceptions.
