Monitoring
The quarterly dashboard is green because TRIR fell and every scheduled mixer-interlock check was completed. During a field verification, three interlocks fail their stop function and one remains bypassed at shift change. You are the CSP. Which evidence should control the residual-risk decision?
Updated . Independent CSP-11 practice. Not BCSP-endorsed. No pass guarantee.
Decision rule
Monitor whether the control still works, not only whether harm arrived.
Inspections that never close defects are theater.
In 20 seconds
- Monitor whether the control still works, not only whether harm arrived.
- Monitoring verifies controls and risk assumptions still hold.
- Leading checks on critical controls beat lagging-only surprise.
- Inspection quality matters — check-box tours fail.
How to apply Monitoring
THEORY: Leading control-health vs lagging harm. KRIs need thresholds and owners. CALC: If a stem gives defect close-out %, use it. Do not invent SPC limits. FIELD: Interlock tests 40% overdue. That is the monitor result — fix tests, not the slogan. ASSESS: Watching TRIR instead of bypass hours is the trap.
Worked case
Wrong first move: Wait for the next injury. Correct move: Act on overdue tests and open defects.
Why the rule holds
Related: 26, 43, 106.
Same rule, different setting
A VP wants monitoring to be “TRIR plus the insurance renewal.” The well-control barrier has not been function-tested in 18 months. You are the CSP. The question is what the risk loop actually monitors.
A VP wants monitoring to be “TRIR plus the OCIP renewal.” Leading-edge anchors have not been inspected since last season. You are the CSP. The question is what the risk loop actually monitors.
Picture / model
Monitor the functions that keep a hazardous scenario controlled, distinguish completion metrics from performance evidence, and reopen treatment when a safeguard, assumption, or operating condition no longer supports the accepted residual-risk decision.
Exam traps
Original SafePath contrasts for Monitoring. These are study traps, not recalled BCSP items.
| Trap | Correct read |
|---|---|
| TRIR / DART | Lagging site rates — not barrier health for a named control |
| Function test / inspection | Monitoring that the Treat still works |
| Premium / deductible | Financing movement — not residual energy |
| 1904 clocks | Legal notification — not the risk-process Monitor step |
Field move
A try-out and defeat sample shows whether the new interlock works.
Scenarios
Scenario 1 · Fundamental · CSP-11 D3.04
You are the CSP reviewing a live decision at an offshore fabrication night shift. Operations proposes a convenience path. Which monitoring evidence best shows that a newly installed mixer interlock functions as intended?
Best action: Logged function tests / try-out and defeat sampling against the residual assumption.
Why: Evaluate the best response. The new mixer interlock needs evidence that it functions. Logged function tests, try-outs, and defeat sampling check the residual assumption. A rate, a premium, or a poster count does not. Annual TRIR only. — Annual TRIR is a lagging site rate. It does not show that this…
Scenario 2 · Applied Scenario · CSP-11 D3.04
A camera used as “detection monitoring” has been dark for 6 weeks because of a utility outage. Team still paints Detection healthy. Issue?
Best action: Utilities/power to the control are part of Monitor; a dark camera is not detection.
Why: Evaluate the best response. The camera used as detection has been dark for six weeks after a utility outage, and the team still paints detection healthy. Utilities and power are part of the monitor. A dark camera is not detection. None — cameras are inherent. — Cameras are not inherent safety. A de…
Scenario 3 · Advanced Engineering · CSP-11 D3.04
Deductible raised (retain) AND overdue deluge inspection ignored (reduction not monitored). What two strategies are in play, and the Monitor miss?
Best action: Retain without monitoring Reduce: financing changed, severity control health unknown — inspection is the Monitor.
Why: Evaluate the best response. The deductible was raised and the overdue deluge inspection was ignored. That is retain without monitoring the reduction barrier. The inspection is the monitor that was skipped. Avoid + reduce, fully monitored. — Calling the pair avoid plus reduce, and fully monitored, m…
If the stem says this
| Stem signal | Best direction |
|---|---|
| critical safeguard is never proof-tested | add monitoring of that control |
| monitoring finds failure but no CAPA | open corrective action |
| only TRIR is monitored | add leading control-health measures |
| two answers work | higher hierarchy / system / design |
| MoC changed the barrier | reset monitoring criteria |
Check before you leave
- ✓ Monitoring verifies controls and risk assumptions still hold.
- ✓ Leading checks on critical controls beat lagging-only surprise.
- ✓ Inspection quality matters — check-box tours fail.
- ✓ Triggers for re-assessment include MoC, incidents, and threshold breaches.
- ✓ Independence of monitoring from the operator of the control strengthens assurance.
- ✓ Corrective action must follow failed monitoring.
